DECLARATION AND AUTHORIZATION

I declare that the information provided on this form is true and complete.

I authorize the organizers of MISS WOMAN SUMMER INC (MWS) to use my image, name, voice, and any audiovisual material related to me in publications, digital media, and social media for promotional purposes, without additional compensation, in accordance with the Illinois Right of Publicity Act (765 ILCS 1075/).

I authorize MWS and its assignees and licensees to photograph, record videos, and record my voice. Furthermore, I authorize MWS to use these images, videos, and recordings, as well as my name, voice, and likeness, as follows:

Use of these materials may include, but is not limited to, reproduction, distribution, creation of derivative works, public display, and performance.
Use may be in modified or composite formats and in any media, whether now known or hereafter developed, including the official MWS website and social media accounts.
This use may be for any purpose, anywhere in the world, and indefinitely, including educational, commercial, advertising, and promotional purposes.
I acknowledge that I will not receive any compensation for the use of these materials and that MWS has exclusive rights to the images, videos, recordings, and any derivative works created from them. I waive the right to inspect or approve the use of any printed or electronic copies.

I hereby release MWS, its assignees, and licensees from any and all claims that may arise from such uses, including, but not limited to, claims for defamation, invasion of privacy, infringement of publicity rights, moral rights, or copyright.

I declare that I am eighteen (18) years of age or older and that I have read this release and fully understand its contents. I understand that this release is binding on me, my heirs, assigns, and estate. MWS is not obligated to exercise any of the rights granted under this release. This release represents the complete and exclusive agreement between the parties.

I further understand that my participation is voluntary and does not create an employment relationship. It is subject to the terms and conditions of the contest rules. I also authorize the use of my personal information solely for event administrative purposes, in compliance with the Illinois Personal Information Protection Act (815 ILCS 530/).
